About the Role
A full-time opportunity is available in Shepparton, Victoria for a compassionate and detail-oriented Care Support Officer to become a valued member of our care team. This role is responsible for conducting AN-ACC assessments, developing and maintaining individualised care plans, and championing resident-centred care practices. The successful candidate will work in close partnership with the care team, residents, families, and the Facility Manager to drive high-quality outcomes through evidence-based assessments and thoughtful care strategies.

Key Responsibilities

  • Conduct AN-ACC Assessments in collaboration with care staff and leadership.
  • Design and implement best-practice care plans that reflect individual needs and values.
  • Undertake Resident of the Day (ROD) reviews alongside the Facility Manager.
  • Ensure assessments are completed within 72 hours of admission and updated as required.
  • Consult with residents, families, Div 1 nurses, and external providers to guide care planning.
  • Lead continuous improvement initiatives through audits and feedback loops.
  • Support carers and deliver emotional, social and clinical support to residents.
  • Manage specialised care areas including mobility, nutrition, wound care, pressure care, continence etc.
  • Maintain thorough documentation and communication across care teams and families.
  • Complete HELF Checks and coordinate required training with PAL and P&C for new staff.
Essential skills and attributes (selection criteria):
  • Registered Division 1 Nurse with current National Practicing Registration
  • Excellent problem-solving and care planning capabilities
  • Strong working knowledge of the needs of the elderly person residing within a residential care facility.
  • Exceptional problem solving, assessment and care planning skills
  • Excellent AN-ACC operational knowledge
  • Self-motivated, proficient with computerised documentation systems and records
  • Strong interpersonal skills with the ability to build and maintain working relationships within a multi-disciplinary team
